Payment by Mail

Payment by mail can be done in two easy steps:

1. Complete the Online Registration Form
    or print out and complete the Printable  
    Registration Form.
2. Mail payment with your Registration Form to:
     Bodyscapes, 3235 Vollmer Rd. Suite 204, Flossmoor, IL 60422.
    
Checks should be made out to Bodyscapes.
A printed Registration Form is not necessary if you submit the Online Registration Form.

Payment by Phone
If you wish to make your payment by phone, a credit card is required. A Registration Form will also need to be submitted. Visa and Master Card are accepted credit cards.

Pre-Registration Discounts
Pre-registration discounts will be honored only if payment is received on or before the discount deadline.
